Ich vs Velvet vs Epistylis: Three Look-Alikes
Ich, velvet, and epistylis all put pale specks on a betta, and all three need a different response. Ich looks like uniform salt grains sitting flat in the skin. Velvet looks like fine gold or rust dust and usually needs a flashlight to see. Epistylis looks fuzzy, irregular, and stands out from the body.
Getting this wrong costs fish. The standard ich playbook, crank the heat and dose an antiparasitic, is actively dangerous if what you are looking at is epistylis. The standard velvet treatment does nothing for a bacterial problem. Before you medicate, spend five minutes on the identification.

Ich: salt grains embedded in the skin
Ich is Ichthyophthirius multifiliis, a single celled parasite. The white spot you see is the feeding stage, sitting under the outer skin layer. That is why the spots look flat and well defined rather than fuzzy, and it is also why they are so uniform: they are all the same organism at the same size, roughly 0.5 to 1 mm across.
The behavior matters as much as the spots. A betta with ich typically flashes, meaning it rubs and darts against decor, substrate, or the glass. You may see that flashing a day or two before any spot is visible.
The critical thing to understand about ich is that the visible stage is the untreatable stage. While the parasite is embedded in the fish, medication cannot reach it. It only becomes vulnerable after it drops off, encysts, divides, and releases free swimming offspring. That whole cycle runs fast in warm water and slowly in cool water, from under a week to several weeks. Two consequences follow. First, spots disappearing does not mean you are done. Second, you have to keep treating for several days after the last spot is gone, or the next wave reinfects the fish.
Velvet: gold dust you need a flashlight to see
Velvet is caused by Piscinoodinium, a dinoflagellate. It is a genuinely different kind of organism from ich: it contains chlorophyll, which is what gives it the gold or rust color and which means it partly feeds on light as well as on the fish. Two practical facts fall directly out of that biology. Darkness works against it, and it is often on the gills doing damage well before you can see anything on the body.
The diagnostic move is simple. Turn off the room lights, wait a moment, and shine a flashlight across the fish at an angle. Velvet shows up as a fine metallic sheen, like the fish has been dusted with powder. In normal tank lighting on an already iridescent betta, it is very easy to miss.
Bettas are among the fish considered especially susceptible to velvet, and it tends to be advanced by the time it is spotted. Look for the behavior alongside the coating: clamped fins, lethargy, loss of appetite, flashing, and labored or rapid breathing. Rapid breathing with very little visible on the body points at gill involvement and means you are already late.
Epistylis: fuzzy tufts that point away from the fish
Epistylis is the one most people have never heard of, and it is the one most often misread as ich. It is a colonial organism that anchors itself on a fish by a stalk. Here is the part that changes everything about treatment: it does not eat the fish. It eats bacteria. The fish is a perch, not a meal.
Visually, look for three tells against ich. Epistylis is translucent or off white rather than the bright, chalky white of ich. It is irregular, with spots of noticeably different sizes in a patchy distribution rather than an even scatter. And it stands out from the fish into the water, sometimes visibly tufted, where ich is embedded and nearly flush.
The bigger tell is context. Epistylis shows up where there is a heavy organic load in the water and, very often, where there is already a bacterial infection on the fish for it to feed on. If you are seeing pale tufts along with ulcers, red patches, ragged skin, or a fish that is declining much faster than a light ich infestation would explain, the epistylis is a symptom. The bacterial infection underneath is the thing that kills.
The mistake that kills bettas
The reflex when white spots appear is to raise the temperature. For ich, that is defensible within limits. For epistylis, it is close to the worst thing you can do, because warmth accelerates the bacterial growth that epistylis lives on and that is doing the real damage.
Even for confirmed ich, we do not endorse the common advice to push a betta to 86 degrees. Our canonical range is 78 to 80 degrees Fahrenheit as ideal, with 76 to 82 acceptable. Sustained temperatures above 82 raise metabolic demand and are taxing on a betta, warmer water holds less dissolved oxygen, and the parasite may already be damaging gill tissue. Pushing an oxygen starved fish with compromised gills into hot water to speed up a parasite is a bad trade.
The position we take: if you are treating ich, raise no higher than 82, move slowly rather than in one jump, add surface agitation so oxygen does not become the limiting factor, and let the medication do the work rather than the heat. If you are not certain the problem is ich, do not raise the temperature at all.
What to do before you medicate anything
Most betta illness starts with an environmental stressor, and medication does not remove a stressor. Correcting water and temperature resolves a lot of what looks like disease, and medicating without fixing the environment usually fails or helps only temporarily. The exception is a fast moving infection, which needs medication alongside the correction rather than after it.
Run these first, in this order:
- Test the water. Target ammonia at 0 ppm, nitrite at 0 ppm, and nitrate under 20 ppm. Any ammonia or nitrite above zero is doing damage on its own.
- Do a large water change if anything is off. Half to three quarters, temperature matched to the tank, always dechlorinated with a conditioner that handles both chlorine and chloramine. Re-test until ammonia and nitrite read zero.
- Check the temperature and stabilize it. Confirm with a thermometer rather than a heater dial.
- Reduce the organic load. Vacuum the substrate, remove uneaten food, and clean the filter media in tank water. This matters most for epistylis, where organic waste is the fuel.
If the fish is gasping at the surface, lying motionless on the bottom, showing scales that stand out like a pine cone, growing cotton like tufts, or losing color rapidly, you are past the identification stage and into an emergency. Treating each one
Ich
Use a proven antiparasitic, dose to the tank volume, and continue for several days past the last visible spot. A modest and gradual temperature raise up to 82 degrees shortens the cycle. Increase surface agitation. Clean the substrate between doses, since that is where the cysts settle and divide.
Velvet
Copper based medications are the standard treatment, and bettas are sensitive to copper, so measure the tank volume properly and dose exactly. Cover the tank or keep the lights off through the course, since the parasite depends partly on light. Epistylis
Do not raise the temperature. Fix water quality aggressively, cut the organic load, and treat the fish for the bacterial infection underneath if there are signs of one, such as ulcers, red or open patches, or rapid decline. Aquarium salt is the appropriate salt for external issues, and Epsom salt is for internal swelling and constipation. They are different compounds and are not interchangeable.
For any of these, a hospital tank makes treatment far easier to control: small, roughly 2.5 to 5 gallons, held near 78 degrees, gently aerated, bare with no substrate or plants, with large daily water changes.
When you honestly cannot tell
Here is the part most guides leave out. Ich and epistylis cannot be separated with certainty by eye. Distinguishing them definitively requires a skin scraping under a microscope, which almost no home keeper is doing. Experienced keepers and aquatic veterinarians disagree about the right approach when the diagnosis is uncertain, and anyone who tells you a photograph settles it is overselling.
What is defensible when you are not sure:
- Fix the environment first. That helps every one of the three and harms none of them.
- Do not raise the temperature. Heat helps ich treatment somewhat and helps epistylis a great deal, and the asymmetry of that risk is not in your favor.
- Watch the response. Ich responds visibly to a correct antiparasitic course within about a week. Epistylis does not improve, or gets worse, on the same treatment. Lack of response is diagnostic information.
- Judge by trajectory, not by spot count. A fish declining fast with irregular spots is far more likely dealing with a bacterial infection than a light parasite load.

When it is none of the three
Pale patches on a betta are not always a parasite. True fungal growth looks like cotton wool: soft, irregular, localized, and usually anchored to a wound or a spot of previous damage. Columnaris is bacterial, often appears as saddle shaped or grey white patches, moves quickly, and is regularly mistaken for fungus, which matters because the treatments are unrelated. Breeding tubercles, sand or debris caught in mucus after flashing, and healing skin damage all produce pale marks that are not disease at all.
Preventing all three
The routine that avoids the large majority of these problems is unglamorous and short: test the water weekly, do regular partial water changes of roughly 25% to 30%, always use a conditioner that neutralizes both chlorine and chloramine, keep the tank properly cycled, and quarantine anything new before it goes near an established fish. How can I tell epistylis from ich without a microscope?
You cannot do it with certainty, but the odds shift on three visual cues and one contextual one. Ich is bright white, uniform in size, and embedded nearly flush with the skin. Epistylis is translucent, irregular in size, and protrudes into the water. The context cue is the tank: heavy organic load and signs of bacterial infection point toward epistylis.
Does velvet always look gold?
It is usually described as gold or rust, but it can read as grey or greenish depending on the fish and the angle of light. The more reliable identifier is texture and behavior: a fine even dusting rather than discrete spots, plus clamped fins, lethargy, and labored breathing. Check it with a flashlight in a dark room.
Can I cure ich with heat alone?
Not safely on a betta. The heat only speeds the parasite through its cycle, and the temperature at which it stops reproducing reliably is higher than we recommend keeping a betta. Use medication as the treatment and a modest temperature raise as support.
Will aquarium salt treat any of these?
Aquarium salt is a supportive treatment for external problems and can help alongside a real treatment. It is not a cure for velvet, and treating a suspected internal or bacterial problem with salt alone wastes time the fish may not have. Is any of this contagious to my other fish?
Ich and velvet are both highly contagious and will move through a tank. Velvet can also travel between separate tanks on the spray from air pumps and filters. Epistylis is best thought of as a symptom of tank conditions rather than something a fish catches from a neighbor, which means other fish in the same water are at risk from the same conditions.
How long should treatment run?
Longer than the spots last. For ich, the visible stage is the stage medication cannot touch, so continue several days past the last visible spot to catch the next generation.
